SUMMARY:

The Department of Defense has submitted to OMB for clearance, the following proposal for collection of information under the provisions of the Paperwork Reduction Act (44 U.S.C. Chapter 35).

Type of Request: New Collection.

Number of Respondents: 5,000.

Responses per Respondent: 1.

Annual Responses: 5,000.

Average Burden per Response: 10 minutes.

Annual Burden Hours: 833.

Needs and Uses: The information from the DD X656 is used to annotate employees understanding of the background check requirements and obtain information in regards to any criminal history that might prevent an individual from being employed in a CDP.

Affected Public: Individuals or households.

Frequency: On occasion.

Respondent's Obligation: Required to Obtain or Retain Benefits.
