    Title: The National Outcomes Survey of People with Developmental 
Disabilities.
    OMB No.: New.
    Description: This survey will focus on educational, employment, 
housing, and health situations of people with developmental 
disabilities, and their opportunities to make choices. Respondents will 
be people with disabilities and their family members. They will also be 
queried regarding assistance from three specific public entities.
    Respondents: Individuals or Household.
