Notice of Issuance of Revised NRC Form 3; Notice to Employees

    The Nuclear Regulatory Commission has issued a revised NRC Form 3, 
``Notice to Employees'', dated September 1998, effective October 7, 
1998. The Form has been revised to reflect the closure of the NRC field 
office located in Walnut Creek, California, effective close of 
business, September 30, 1998. Individuals who have been reporting 
concerns to the Walnut Creek field office should now report their 
concerns to the NRC's Region IV office located in Arlington, Texas. The 
toll-free number for the Arlington, Texas office is [phone].
    A copy of NRC Form 3 has been placed in the NRC's Public Document 
Room, the Gelman Building, 2120 L Street, NW. (Lower Level), 
Washington, DC 20037, for review and copying by interested persons.
