The Department of Treasury has submitted the following public
information collection requirement(s) to OMB for review and clearance

Special Request: In order to conduct the survey described below in
January 1997, the Department of the Treasury is requesting that the
Office of Management and Budget (OMB) review and approve this
information collection by November 4, 1996. To obtain a copy of this

Type of Review: Revision.
Title: Installment Agreement Customer Satisfaction Survey.
Description: In July 1994 the Acting Regional Inspector Southwest
for Internal Audit reported that there were some weaknesses in the
streamlined installment agreement process that was implemented during
the 1993 filing season. The streamlined installment agreement allowed
taxpayers to request payment of their taxes through an installment
agreement by attaching a

Form 9465 at the time they filed their tax return. A review of
installment requests in the Ogden, Austin and Fresno Service Centers
revealed that installment agreements are being granted even though the
taxpayer may fail to under the cause of the balance due or what actions
need to be addressed to prevent the situation from recurring in the
future.
The purpose of this survey is to determine the reasons taxpayers
request installment agreements and what role the streamlined
installment agreements can have in helping taxpayers meet their tax
obligations in the future.

Estimated Number of Respondents: 1,650.
Estimated Burden Hours Per Respondent: 5 minutes.
