ACTION: Notice.

    The Department of Defense has submitted to OMB for clearance, the 
following proposal for collection of information under the provisions 
of the Paperwork Reduction Act of 1995 (44 U.S.C. Chapter 35).
    Title: Department of Defense Khamisiyah Survey.
    Type of Request: New collection; Emergency Processing requested 
with shortened public comment period ending December 7, 1996. An 
approval date of December 24, 1996 is requested.
    Number of Respondents: 13,783.
    Responses Per Respondent: 1.
    Annual Responses: 13,783.
    Average Burden Per Response: 7 minutes.
    Annual Burden Hours: 1,608 hours.
    Needs and uses: This collection of information is necessary to 
facilitate investigation of the events surrounding the demolition of 
munitions near Khamisiyah, Iraq from 1-15 March 1991 (immediately after 
the Gulf War). This survey is part of our ongoing effort to better 
understand the events during and after the Gulf War, and potential 
health consequences. Respondents are current and former members of the 
US military who were within 50 kilometers of the munitions storage area 
near Khamisiyah, Iraq.
    Affected Public: Individuals or households.
    Frequency: One time.
    Respondent's Obligation: Voluntary.
