Title of Form: Learn and Serve America--Higher Education Annual 
Accomplishments Survey and Community Impact Survey
Need and Use: The National and Community Service Trust Act of 1993 
(Pub. L. 103-82) requires the Corporation for National Service to 
evaluate its programs on a regular basis. This information is required 
for program management, planning, and required record keeping
Type of Request: Submission of a new collection
Respondents Obligation to Reply: Voluntary
Frequency of Collection: Once each year for three years
