AGENCY: Veterans Benefits Administration, Department of Veterans

ACTION: Notice.

SUMMARY: The Veterans Benefits Administration (VBA), Department of
Veterans Affairs, has submitted to the Office of Management and Budget
(OMB) the following proposal for the collection of information under
the provisions of the Paperwork Reduction Act (44 U.S.C. Chapter 35).
OMB Control Number: 2900-0067.
Title and Form Number: Application for Automobile or Other
Conveyance
[[Page 5072]]
and Adaptive Equipment, VA Form 21-4502.
Type of Review: Extension of a currently approved collection.
Need and Uses: The form is used to gather the necessary information
to determine if the veteran has established entitlement to an
automobile allowance or adaptive equipment.
Affected Public: Individuals or households.
Estimated Total Annual Burden: 375 hours.
Estimated Total Average Burden Per Respondent: 15 minutes.
Frequency of Response: On occasion.
Estimated Total Number of Respondents: 1,500.
