SUMMARY:
In compliance with the Paperwork Reduction Act (PRA) of 1995 (44 U.S.C. 3501-21), this notice announces that the Veterans Health Administration (VHA), Department of Veterans Affairs, will submit the collection of information abstracted below to the Office of Management and Budget (OMB) for review and comment. The PRA submission describes the nature of the information collection and its expected cost and burden, and includes the actual data collection instrument.
DATES:
Comments must be submitted on or before March 6, 2008.

Title: Nation-wide Customer Satisfaction Surveys, VA Forms 1465-1 through 1465-4.
OMB Control Number: 2900-New (VA Form 1465-1).
Type of Review: New collection.
Abstract: The purpose of the Survey of Health Experience of Patients (SHEP) Survey is to systematically obtain information from VA patients to identify problems or complaints that need attention and to improve the quality of health care services delivered to veterans. Data will be use to measure improvement toward the goal of matching or exceeding the non-VA external benchmark performance in providing quality health care services to veterans.
An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number. The Federal Register Notice with a 60-day comment period soliciting comments on this collection of information was published on October 24, 2007, at pages 60406-60407. Start Printed Page 6770
