This is a Notice of the Presidential declaration of a major disaster for Public Assistance Only for the State of Indiana (FEMA-4058-DR), dated 03/22/2012.

Incident: Severe Storms, Straight-line Winds, and Tornadoes.

Incident Period: 02/29/2012 through 03/03/2012.

Effective Date: 03/22/2012.

Physical Loan Application Deadline Date: 05/21/2012.

Economic Injury (EIDL) Loan Application Deadline Date: 12/24/2012.
