AmeriCorps*NCCC's Proposal for an Information Collection Form
AGENCY: Corporation for National and Community Service.
ACTION: Information collection request submitted to the Federal Office
of Management and Budget (FOMB) for review.

SUMMARY: This notice provides information about an information
collection proposal by AmeriCorps*National Civilian Community Corps
(NCCC), currently under review by the Office of Management and Budget
(OMB).
DATES: OMB and AmeriCorps*NCCC will consider comments on the proposed
collection of information and recordkeeping requirements received
before May 12, 1995. Copies of the proposed forms and supporting
documents may be obtained by contacting AmeriCorps*NCCC.

SUPPLEMENTARY INFORMATION: Section 3517 of the Paperwork Reduction Act
of 1980 (44 U.S.C. Chapter 3517) requires that the Director of OMB
provide interested Federal agencies and persons an early opportunity to
comment on information collection requests.

Title of Forms: Applicant Medical Prescreening Form.
Need and Use: This information is used for program management,
planning, and required recordkeeping.
Type of Request: Submission of new collection.
Respondent's Obligation to Reply: Required to receive benefits.
Frequency of Collection: One time per selected applicant.
Estimated Number of Response: 2,000.
Average Burden Hours Per Response: .5 hours (reporting and
recordkeeping).
Estimated Annual Reporting or Disclosure Burden: 500 hours.
Regulatory Authority: The National and Community Service Act of 1990,
as amended.
