Public Information Collection Requirement Submitted to the Office
of Management and Budget for Clearance
AGENCY: Administration on Aging.
The Administration on Aging, Department of Health and Human
Services, is submitting the following proposal for the collection of
information in compliance with the Paperwork Reduction Act (Public Law
96-511); Title VI Program Performance Reports.
Type of Request: ``Reinstatement, without change.''
Use: To continue an existing information collection, Title VI
Program Performance Report, from Title VI grantees to use in reporting
information on programs funded by Title VI as required under Section
202(a)(19), Section 614(a)(2), and Section 614(a)(3) of the Older
Americans Act, as amended.
Frequency: Semiannually.
Respondent: Tribal organizations and nonprofit organizations
representing Native Hawaiians.
Estimated number of responses: 222.
Estimated Burden Hours: 648.
Additional Information or Comments: The reporting system would
become effective in fiscal year 1997. The reporting form would include
the following elements:
Total number services provided;
Unduplicated number of persons receiving services;
Staffing, including volunteers;
Number of meal sites operated during budget period; and
Total number of resources used to support the Title VI
programs.
