    The Department of Treasury has submitted the following public

    Title: EP/EO Determination Centralization Customer Satisfaction 
Survey.
    Description: In order to ascertain whether the centralized 
determination process is of value, IRS will administer this survey 
twice for each key district office.
    Respondents: Business or other for-profit.
    Estimated Number of Respondents: 16,980.
    Estimated Burden Hours Per Respondent: 3 minutes.
    Frequency of Response: Other.
    Estimated Total Reporting Burden: 849 hours.
