Title: Customer Service Survey (Evaluation Questionnaire)

Abstract: Executive Order 12862 requires agencies to survey customers 
to determine the kind and quality of services they want and their level 
of satisfaction with existing services. The information supplied by 
this survey will determine customer satisfaction with OSM services and 
to identify areas needing improvement.

Description of respondents: Coal-field residents; environmental, 
citizen and industry groups; coal operators; and regulatory authorities
