and Revision of SF 526, Medical Record--Interstitial/Intercavitary 
Therapy

AGENCY: General Services Administration.

ACTION: Notice.

SUMMARY: The General Services Administration/ICMR is changing the 
stocking requirement of SF 526, Medical Record--Interstitial/
Intercavitary Therapy. This form is now authorized for local 
reproduction. You can request camera copy of SF 526 from General 
Services Administration (CARM), Attn.: Barbara Williams, (202) 501-
0581.
    This form also is revised to:
    To delete ``grade'' and ``date'' from ``PATIENT'S IDENTIFICATION'' 
item and replace with ``ID No. (SSN or other);''.
