SUMMARY: In accordance with the Paperwork Reduction Act of 1980 (44 
U.S.C. chapter 35), the Railroad Retirement Board has submitted the 
following proposal(s) for the collection of information to the Office 
of Management and Budget for review and approval.

Summary of Proposal(s)

    (1) Collection title: Survivor Questionnaire.
    (2) Form(s) submitted: RL-94-F.
    (3) OMB Number: 3220-0032.
    (4) Expiration date of current OMB clearance: Three years from date 
of OMB approval.
    (5) Type of request: Extension of the expiration date of a 
currently approved collection without any change in the substance or in 
the method of collection.
    (6) Frequency of response: On occasion.
    (7) Respondents: Individuals or households.
    (8) Estimated annual number of respondents: 26,500.
    (9) Total annual responses: 26,500.
    (10) Average time per response: .17377 hours.
    (11) Total annual reporting hours: 4,605.
    (12) Collection description: Under section 6 of the Railroad 
Retirement Act, benefits are payable to the survivors or the estates of 
deceased railroad employees. The collection obtains information about 
the survivors if any, payment of burial expenses and administration of 
estate when unknown to the Railroad Retirement Board. The information 
will be used to determine whether and to whom benefits are payable.
