Title: Welfare Reform Demonstration: Special Application Form.
OMB No.: 0970-0134.
Description: The form will be used by State welfare agencies to
apply for federal waivers under the 30-day waiver approval process
proposed by the President in his July 31, 1995, speech to the National
Governors' Association. Under the process, requests for waivers of
federal law for welfare demonstration projects falling within any of
five broad policy areas will be approved by the federal government
within 30 days of receipt of this request.
