Enhancing Partnerships With Private Sector Health Care Provider
Organizations; Notice of Availability of Funds for Fiscal Year 1995

    Introduction

The Centers for Disease Control and Prevention (CDC) announces the
availability of fiscal year (FY) 1995 funds for cooperative agreements
to enhance partnerships with private sector health care provider
organizations and to promote the improvement of immunization coverage
    in primary care settings.
The Public Health Service (PHS) is committed to achieving the
health promotion and disease prevention objectives of Healthy People
2000, a PHS-led national activity to reduce morbidity and mortality and
improve the quality of life. This announcement is related to the
priority areas of Immunization and Infectious Diseases. (To order a
copy of Healthy People 2000, see the section Where to Obtain Additional
    Information.) [[Page 25237]]

    Authority

This program is authorized under the Public Health Service Act,
    Sections 301 [42 U.S.C. 241] and 317 [42 U.S.C. 247b], as amended.

    Smoke-Free Workplace

PHS strongly encourages all grant recipients to provide a smoke-
free workplace and to promote the nonuse of all tobacco products, and
Public Law 103-227, the Pro-Children Act of 1994, prohibits smoking in
certain facilities that receive Federal funds in which education,
library, day care, health care, and early childhood development
    services are provided to children.

    Eligible Applicants

Eligible applicants for this program are national private sector
health care provider organizations and associations with an active
membership of at least 2500 health care providers.

    Availability of Funds

Approximately $500,000 is available in FY 1995 to fund up to seven
awards. The average award will range from $75,000 to $150,000. Awards
will begin on or about September 1, 1995, and will be made for 12 month
budget periods within a project period of up to 5 years. Funding
estimates outlined above may vary and are subject to change.
Continuation awards within the approved project period will be made on
the basis of satisfactory progress, an acceptable application, and the
    availability of funds.

    Purpose

The purpose of this cooperative agreement is:
        1. To establish partnerships with national private provider
organizations and associations to effectively utilize the combined
resources of the public and private health care delivery systems.
        2. To meet the immunization objectives established by the Secretary
of the Department of Health and Human Services. Priority will be given
