    Agency Information Collection Activities: Submission for OMB 
    Review; Comment Request
    
        Periodically, the Substance Abuse and Mental Health Services 
    Administration (SAMHSA) will publish a list of information collection 
    requests under OMB review, in compliance with the Paperwork Reduction 
    Act (44 U.S.C. Chapter 35). To request a copy of these documents, call 
    the SAMHSA Reports Clearance Officer on (301) 443-0525.
        FY 1997/1998 Substance Abuse Prevention and Treatment Block Grant 
    Application Format--Revision of a currently approved collection--The 
    Public Health Service Act (42 U.S.C. 300x 1-9) authorizes block grants 
    to States for the purpose of providing prevention and treatment 
    services.

    Under the provisions of the law, States may receive allotments only 
    after an application is approved by the Secretary, DHHS. The uniform 
    application format provides States with the forms and instructions for 
    their applications so they can comply with the requirements of the law 
    and regulations implementing the law. The annual burden estimate is 
    shown below:
