Request for Clearance of a Revised Information Collection Form RI
25-15
AGENCY: Office of Personnel Management.
ACTION: Notice.

SUMMARY: In accordance with the Paperwork Reduction Act of 1980 (title,
44 U.S. Code, chapter 35), this notice announces a request for
clearance of a revised information collection. Form RI 25-15, Survey of
Student's Eligibility to Receive Benefits, is used to collect
sufficient information from adult children of deceased Federal
employees or annuitants to assure that the child continues to be
eligible for payments from OPM.
Approximately 12,000 RI 25-15 forms are completed annually. It
takes approximately 15 minutes to complete this form. The total annual
burden is 3,000 hours.
