AGENCY: Federal Deposit Insurance Corporation.
ACTION: Notice of Information Collection submitted to OMB for review
and approval under the Paperwork Reduction Act of 1980.

SUMMARY: In accordance with requirements of the Paperwork Reduction Act
of 1980 (44 U.S.C. Chapter 35), the FDIC hereby gives notice that it
has submitted to the Office of Management and Budget a request for OMB
review for the information collection system identified below.
Type of Review: Extension of expiration date without any change in
substance or method of collection.
Title: Application for a Merger or Other Transaction Pursuant to
Section 18(c) of the FDI Act (Phantom or Corporate Reorganization).
Form Number: FDIC 6220/07.
OMB Number: 3064-0015.
Expiration Date of Current OMB Clearance: August 31, 1995.
Frequency of Response: On occasion.
Respondents: State nonmember banks wishing to effect a merger-type
transaction through corporate reorganization or phantom merger.
Number of Respondents: 100.
Total Annual Responses: 100.
Total Annual Burden Hours: 2,000.
