Collection

summary: In accordance with the Paperwork Reduction Act of 1980 (title
44, U.S. Code, chapter 35), this notice announced a request for review
of a currently approved information collection. Form DPRS-2809, Request
to Change FEHB Enrollment or to Receive Plan Brochures, is used by
former spouses who are eligible to elect, cancel, or change health
benefits enrollment during open season.
Approximately 28,000 forms are completed annually. This form
requires approximately 10 minutes to complete. The annual burden is
4,700 hours.
