Form 2809-EZ2
AGENCY: Office of Personnel Management.
ACTION: Notice.

SUMMARY: In accordance with the Paperwork Reduction Act of 1980 (title
44, U.S. Code, chapter 35), this notice announces a request for
reclearance of an information collection. OPM Form 2809-EZ22, Open
Season Health Benefits Enrollment Change Form, is used by annuitants
only at Open Season to elect a change in health benefits coverage.
Approximately 36,559 OPM Forms 2809-EZ2 are completed annually.
Each form takes approximately 30 minutes to complete. The annual burden
is 18,280 hours.
