Submission for OMB Review; Comment Request

    Title: At-risk child care program, Annual Report, ACF-301.
    OMB No.: 0970-126.
    Description: Completing this form partially fulfills the reporting 
requirements of Section 402(i)(6)(B) of the Social Security Act (the 
Act) whereby the Secretary of DHHS must compile and report certain 
information annually that is pertinent to the At-Risk program. 
Additionally the report provides a means for each State to report 
subsequent period expenditures to determine the State's compliance with 
the non-supplantation provisions found at Section 402(i)(5)(D) of the 
Act.
