    Title: Customer Satisfaction Survey.
    Agency Form Number: None.
    OMB Approval Number: None.
    Type of Request: New collection.
    Burden: 1,750 hours.
    Number of Respondents: 7,000.
    Avg Hours Per Response: 15 minutes.
    Needs and Uses: This survey will be used by the Department of 
Commerce to obtain feedback and information from Commerce customers in 
order to make quality improvements to our products and services.
    Affected Public: Individuals, state or local governments, 
businesses or other for-profit institutions, non-profit institutions, 
and small businesses or organizations.
    Frequency: One-time.
    Respondent's Obligation: Voluntary.
