summary: In accordance with the Paperwork Reduction Act of 1980 (title
44, U.S. Code, chapter 35), this notice announces a request for
reclearance of an information collection. Form DPRS 2809, Request to
Change FEHB Enrollment or to Receive Plan Brochures, is used by former
spouses who are eligible to elect, cancel, or change health benefits
enrollment during open season.
Approximately 15,000 forms are completed annually. The form
requires approximately 10 minutes to complete. The annual burden is
2,500 hours.
