Common Carrier Bureau Announces Release of September Version of 
Telecommunications Reporting Worksheet (FCC Form 499-S) for 
Contributions to the Universal Service Support Mechanisms

AGENCY: Federal Communications Commission.

ACTION: Notice.

SUMMARY: On July 30, 1999, the Common Carrier Bureau released a public 
notice announcing the release of the September version of the 
Telecommunications Reporting Worksheet (FCC Form 499-S) and its 
accompanying instructions. The intended effect of this action is to 
make the public aware of the new worksheet and to remind contributors 
to the universal service support mechanisms of the need to file the 
worksheet on September 1, 1999.

Telecommunications Reporting Worksheet (FCC Form 499-S) and its 
accompanying instructions are attached to the Public Notice. The full
