    Description: This form is prepared by the applicant for payment of 
a Postal Savings Account. This form is used to identify the depositor 
and ensure that payment is made to the proper person. POD Form 315 was 
formerly used by the Post Office Department for processing payments, 
when payments of accounts were their responsibility.
