Incident: Severe Storms, Tornadoes, Straight-line Winds, and Flooding.

Incident Period: 06/26/2014 through 07/07/2014.

Effective Date: 08/19/2014.

Physical Loan Application Deadline Date: 10/06/2014.

Economic Injury (EIDL) Loan Application Deadline Date: 05/05/2015.

All other information in the original declaration remains unchanged.
