The new outpatient measures for CY 2014 addressing diabetes mellitus are defined in the Specifications Manual, however it is unclear what defines a hospital outpatient setting. The manual references E/M codes found in Appendix A, table 11, but this table is missing. It would be helpful to know the E/M codes so that we can determine our patient volume. Please include a table that identifies the measure, the time period for data collection (eg. encounter dates) and impacted payment year. It is unclear if there are any payment penalities for NOT participating with the ASC measures. If there are payment penalities, how are they calculated?
