In reviewing the template for summary of coverage I have a concern about the box for "what is the premium". Most employers have multiple plans that have different premiums depending on whether the person enrolls only themselves, themself and a spouse, themself and children, etc. If we need a separate 6-8 page summary for each premium level we would innundate employees whith a lot of paper. It would make more sense to do one summary for a plan and list the premiums for each tier on one summary.
