Goodrich Service Bulletin 4A4030-25A379, dated August 10, 2009, converts P/N 4A4030-2 or P/N 4A4030-4 to P/N 4A4030-5
Goodrich Service Bulletin 104003-25A380, Revision 2, dated July 7, 2009, converts P/N 104003-1 to P/N 104003-2:
The modifications per the above service bulletins incorporate corrective actions to prevent door stall due slide pack lacing not completely opening.
During the past month there have reports of one P/N 4A4030-5 Escape slide and one P/N 104003-2 escape slide that experienced a door stall due to slide pack lacing not completely opening. In addition, there has been a report of one P/N 104003-2 escape slide that experienced a door stall due to a new failure mode in which the pack failed to drop from the door of the aircraft.
Goodrich has been investigating these occurrences and has not made a final determination on the root cause but anticipate that a reduced overhaul cycle may be required to mitigate future occurrences until a complete and final corrective action is developed.
It is Goodrich’s recommendation that the issuance of the proposed AD be delayed until further notice pending completion of ongoing investigations.
