1) In section 1, the line where the Employee's Signature and Date should be entered needs to be better identified. We often have employees put their signatures above where they are supposed to rather than on the same line as the date.
2) Add instructions at the top of the form such as, "Please do NOT cross or white out information on this form. If a mistake is made, please start over on a different form."
