In question 20, the applicant is asked to provide the dates of any time in J status. However, there is no space provided to do this. If the dates do not need to be listed separately and will be determined by review of the support documents, remove the requirement to provide dates and require inclusion of all DS-2019 forms for time in J status, removing the option of using the "visa stamp" in support. If the dates need to be provided in addition to the support documentation (as would be necessary if the applicant only provided a copy of the "visa stamp" from which program/status dates cannot always be accurately determined), provide a space for the applicant to do so.
