``RIN 2900-AO15, Use of Medicare Procedures to Enter Into Provider Agreements for Extended Care Services.'' To Whom it May Concern: I began my nursing career 35 years ago in the VA Hospital - at that time I was very concerned about the amount of time the Veterans had to spend in the hospital (sometimes months) because of the difficulty in accessing care at home. I am sure great improvements have been made since that time - and the latest proposal is a wonderful move ahead for the VA and for the Veterans needing home health, palliative care or hospice services at home.
