Request for Reclearance of Form SF 2800
AGENCY: Office of Personnel Management.
ACTION: Notice.

SUMMARY: In accordance with the Paperwork Reduction Act of 1980 (Title
44, U.S. Code, chapter 35), this notice announces a request for
reclearance of an information collection. Form SF 2800, Application for
Death Benefits (CSRS), is used to collect information so that OPM can
pay death benefits to the survivors of federal employees and
annuitants.
Approximately 70,000 SF 2800 forms are completed annually. It takes
approximately 30 minutes to complete the form. The annual burden is
35,000 hours
