The statement that CMS believes that "this enhanced communication will result in an improved transition of care from the hospital or post-acute setting to the home health setting" is not true. In fact it has decreased the effectiveness of discharge planning and cost home health agencies a great deal of time tracking these forms. The Face to Face has proved to be a farce. The expansion of this rule is ridiculous.
